Medical and Dental Office Build-Outs in Mobile and Baldwin County

A medical or dental build-out is the most system-heavy interior a commercial contractor builds. Plumbing at every exam room and operatory, medical gas and dental vacuum and air lines, X-ray shielding, ADA clearances everywhere, and finishes that stand up to cleaning.

Wise Builders builds medical and dental suites with licensed specialty subcontractors for the systems that need them: ASSE 6010 certified medical gas installers, a qualified expert for X-ray shielding design, and licensed plumbing, mechanical, and electrical trades. You get one contract and one project manager over all of it.

We hold Alabama General Contractor License #60291. Call (251) 305-7282 before you sign the lease.

What We Build

What a Medical or Dental Build-Out Includes

Your equipment list drives this build-out. We get it from you and your equipment dealer first, then design the rooms around it.

01

Exam Rooms and Operatories

Exam rooms with a hand sink, cabinets, and power where the equipment goes. Dental operatories with floor boxes or chases for water, air, vacuum, and power at each chair.

02

Plumbing and Utilities

Hand sinks in every clinical room, a sterilization area, lab sinks, and the dental compressor and vacuum room, plus floor trenching when the slab needs it.

03

Medical Gas

Oxygen and nitrous oxide piping for dental and outpatient suites, installed by ASSE 6010 certified medical gas installers and verified before use, following NFPA 99.

04

X-Ray Shielding

Lead-lined drywall or other shielding where a qualified expert's shielding design calls for it. Shielding is designed for your equipment and room layout, not guessed.

05

ADA and Patient Flow

Accessible restrooms, doors, hallways, check-in counters, and at least one accessible exam room, with a layout that separates patient and staff traffic.

06

Finishes That Clean

Sheet vinyl or LVT with welded seams where needed, washable wall finishes, solid-surface counters, and ceilings suited to clinical rooms.

Who We Build For

Practices We Build For

Dental Practices

Dentists come to us when they are opening, relocating, or adding chairs.

  • New practices opening a first office in a leased suite.
  • Associates buying a practice and updating the space.
  • Practices adding operatories or converting storage into clinical rooms.
  • Specialists: orthodontics, pediatric dentistry, oral surgery, and endodontics.

Medical and Wellness Practices

Medical clients are usually outpatient practices in retail or office buildings.

  • Primary care, urgent care, and specialty clinics in a leased suite.
  • Physical therapy, chiropractic, and med spa offices.
  • Behavioral health and counseling offices that need sound control.
  • Veterinary clinics converting retail space.

How We Handle Medical Systems

We are a general contractor, not a medical equipment dealer. On every medical or dental job we bring in licensed specialists for the systems with the highest stakes: medical gas, X-ray shielding, and clinical plumbing. Their work is designed, installed, tested, and documented under our contract and our schedule.

Your equipment dealer sets the rough-in requirements for each chair and each room. We build to those drawings and walk the rough-ins with the dealer before the walls close.

What a Medical or Dental Build-Out Costs

Medical and dental build-outs cost more than offices because of the systems inside the walls. The number of chairs or exam rooms, and whether you need medical gas and X-ray, set the budget.

Typical Gulf Coast Cost Ranges

Typical Gulf Coast ranges. Medical examples are 2,500 square feet; dental examples are 2,000 square feet with 5 operatories. Equipment is not included.

ProjectCost per Sq FtExample TotalOn-Site Time
Second-generation medical refreshexisting exam-room plumbing reused$80 to $140$200,000 to $350,0006 to 10 weeks
Primary care from a white box6 to 8 exam rooms, sink in each$150 to $250$375,000 to $625,00010 to 16 weeks
Specialty or imaginglead-shielded X-ray room, procedure room$200 to $325$500,000 to $810,00012 to 18 weeks
Second-generation dentalexisting operatory plumbing reused$60 to $120$120,000 to $240,0006 to 10 weeks
Dental from a white box$150 to $250$300,000 to $500,00012 to 18 weeks
Dental from a dark shell$200 to $300$400,000 to $600,00014 to 20 weeks

Per chair, dental construction typically runs $45,000 to $75,000 per operatory including each chair's share of sterilization, halls, and the front desk. Dental equipment for a 4 to 6 operatory practice adds roughly $150,000 to $280,000, and X-ray shielding for a panoramic or cone-beam room roughly $15,000 to $40,000.

Budget Lines Most Medical Bids Leave Out

Medical and dental jobs carry costs a standard office bid never sees. We price them before you sign.

01

Shielding Report and State Review

Alabama requires X-ray room plans to go to the state before construction, along with the qualified expert's shielding report when one is used. That review has to happen before framing.

02

Medical Gas Testing

Oxygen and nitrous systems need certified installers and independent verification before use under NFPA 99. Both are line items, not favors.

03

Equipment Lead Times

Chairs, sterilization, and imaging equipment can take longer to arrive than the build-out takes to finish. The rough-ins have to match the equipment you ordered, so order early.

Ranges are hard construction costs, adjusted to the Gulf Coast from national 2026 data: Cushman & Wakefield Office and Retail Fit-Out Cost Guides, Terrapin Construction Group 2026 tenant improvement and medical office cost guides, and published dental build-out guides. Furniture, equipment, IT cabling, and signage are not included unless noted.

How It Works

From Walk-Through to Certificate of Occupancy

Every commercial build-out runs through one contract and one project manager. You make decisions; we carry the schedule, the subs, the permits, and the inspections.

01

Step 1. Walk-Through and Site Survey

We walk the space with you and measure it. We check what is already there: the electrical service, HVAC capacity and duct layout, sprinkler heads, fire alarm devices, restrooms, and the slab. Those existing conditions decide the budget on a medical or dental build-out more than the finishes do.

02

Step 2. Written Scope and Fixed Price

You get a written scope of work with every task, allowance, and exclusion listed, plus a fixed price and a schedule. We build the price around your equipment list, so the rough-ins in the walls match the chairs and equipment that arrive.

03

Step 3. Drawings and Life-Safety Plan

Most commercial permits need drawings, and many need a life-safety plan showing exits, emergency lighting, and fire separation. We bring in the architect or engineer, carry their fee in the price, and keep the drawings tied to the budget.

04

Step 4. Permits and Landlord Approvals

We file with the department that has your address: Build Mobile inside city limits, Mobile County or Baldwin County for unincorporated addresses, and the city building department in Daphne, Fairhope, Foley, Gulf Shores, Orange Beach, and the other incorporated cities. We also handle the landlord's approval package and certificates of insurance.

05

Step 5. Build With Weekly Updates

Your project manager runs every trade and every inspection. In occupied centers we follow the landlord's rules for access, noise, and after-hours work. If the owner changes the plan or we open a wall and find a problem, it becomes a written change order you approve before we do the work.

06

Step 6. Final Inspections, Punch List and Closeout

We pass the final building and fire inspections, walk the space with you, and finish the punch list. Closeout includes lien waivers, warranties, and the paperwork your landlord and insurer ask for, so you can get your certificate of occupancy and open.

Why Wise Builders

Why Business Owners and Landlords Hire Us

01

One Point of Contact

You get one project manager from the walk-through to the final inspection. Owner Cody stays involved on commercial jobs, and the office tracks submittals, change orders, and pay applications.

02

One Fixed Price, in Writing

The scope lists every task, allowance, and exclusion. The price changes only through a written change order you sign before the work happens.

03

Built for Small Commercial Spaces

Large commercial contractors often pass on spaces under 5,000 square feet. Spaces under 2,500 square feet are our sweet spot, and the job gets a dedicated project manager, not whoever is free.

The Credentials Behind Every Commercial Job

Wise Builders holds Alabama General Contractor License #60291 from the Alabama Licensing Board for General Contractors. We also hold Alabama Home Builders Licensure Board license #28471, carry general liability and workers' compensation, and are BBB accredited.

Before you sign with anyone, ask for the license number and look it up on the board's website. Ask for a certificate of insurance naming you and your landlord as additional insured. We send ours with the proposal.

FAQ

Frequently Asked Questions About Medical and Dental Build-Outs

How much does a dental office build-out cost?

On the Gulf Coast, a dental build-out typically runs $60 to $300 per square foot before equipment: $60 to $120 when you reuse a second-generation dental office, $150 to $250 from a white box, and $200 to $300 from a dark shell. That works out to roughly $45,000 to $75,000 per operatory in construction cost.

How long does a medical or dental build-out take?

A second-generation medical or dental refresh takes 6 to 10 weeks on site. A new practice from a white box takes 10 to 18 weeks, and a dental office from a dark shell up to 20 weeks, after permits and the state X-ray review.

Who installs the medical gas?

Medical gas piping is installed by ASSE 6010 certified medical gas installers and verified before use, following NFPA 99. We hire and schedule them under our contract.

Do I need X-ray shielding?

If you install X-ray equipment, a qualified expert reviews your equipment and room layout and specifies the shielding. We build what the shielding design calls for, which may be lead-lined drywall or may be nothing extra.

Should I talk to a contractor before I sign the lease?

Yes. Plumbing at every chair, a compressor and vacuum room, and electrical capacity can make one suite cost far more than another. We walk spaces with you before you sign and show you which one costs less to build.

Do you work with my equipment dealer?

Yes. We build to the dealer's rough-in drawings and walk the rough-ins with them before the walls close, so every chair and piece of equipment connects on install day.
